4 The Licensing Context In the 2013 Ontario Budget, following the recommendations of the Auto Insurance Anti-Fraud Task Force, the government committed to take further action to tackle fraud in the auto insurance sector With Bill 65, the government expanded FSCO s mandate to include the licensing and regulation of the business systems and business practices of healthcare service providers that directly invoice auto insurers for statutory accident benefits
5 The Benefits of Licensing Licensing approach Integrity & Honesty Accountability Transparency Compliance
6 Your Role as Principal Representative As Principal Representative, you re responsible for completing your business licence application, as well as for: Ensuring your business and every person authorized to provide specified goods or services on your business behalf complies with the Act Ensuring that your business systems, practices and its management of operations are carried out in accordance with the law and with integrity and honesty Making recommendations to your business regarding changes in its business systems, practices and the management of its operations, as necessary Ensuring that a system of supervision is in place to ensure that all requirements under the law are met Providing attestations on your business behalf regarding its compliance with the Act, as may be required by the Superintendent
7 What You Need Before You Apply Articles of Incorporation or Partnership Agreements (for a corporation or a partnership) An Ontario business registration number, if applicable (typically found on the articles of incorporation or registration of the partnership) Details of trade names registered and used by the business and the expiration date of the related master business licence registration Contact info of all officers, directors or partners of the business including their addresses The business Canada Revenue Agency business number The total number of unique statutory accident benefit claimants in respect of whom the business received payment in 2013 from an automobile insurer for an invoice submitted through HCAI Information about each facility, branch or location registered with HCAI (i.e., names, addresses and registration numbers) to be listed under the business licence The total number of regulated and unregulated practitioners, by profession, to be listed under the business licence
8 Application Review Process Takes Time Do not expect an immediate response from FSCO FSCO must conduct a detailed review of all applications to ensure that only those businesses suitable for a licence are issued a licence; this takes time and includes criminal background checks

10 FSCO s Public Registry By law, FSCO is required to maintain a public registry of current and former licensed service providers Currently lists the legal or trade name, as applicable, of licensed service providers Newly licensed service providers are added weekly. By December 1st, will include all those service providers that applied before August 31 st that were issued a licence Will be expanded to include: Licence numbers Ontario mailing addresses Licence issue dates Licence statuses Licence conditions Previous licence statuses Names of principal representatives Addresses of facilities, branches and/or locations
