2 Sport Liability Insurance- an Overview Why Liability Insurance? Because no matter how careful we are, accidents happen. You can be sued by anyone who claims injury or damages resulting from your activities. You may not be liable, but you will need to be defended in court. A liability policy will pay for his defense as well as any cost awarded against you. Who is Covered? All Registered Clubs and their members duly registered with the including an Officers, Employees, Directors, Coaches, Instructors, Referees, Managers, Trainers, Club officials, Participants and Volunteer, while acting on behalf of the association. Activities Covered? Sanctioned or approved activities governed under the Constitution and By-Laws of Ontario Lawn Bowls Association.

3 Commercial General Liability Insurance Types of Coverage This policy will pay those sums that the insured becomes legally obligated to pay as compensatory damages because of bodily injury to or damage to property of others, such as spectators, passers-by, property of lessors and others resulting from your operations or actions. Policy includes blanket additional insureds endorsement to cover parties such as municipalities, Sponsors, Facility owners. Highlights of coverage Commercial General Liability / per occurrence $5,000,000 Personal Injury and Advertising Injury Liability$5,000,000 Products and Completed Operations $5,000,000 Broad Form Tenants Legal Liability -$1,000,000 Non-Owned Automobile Contingent Employers Liability Host Liquor Liability included Participant Liability Limited World Wide Coverage 60 Day Cancellation Clause OEF 96- Contractual Liability OEF 94-Physical Damage to Non Owned Automobile Blanket Additional insureds- Cross Liability Directors and Officers Liability This Policy covers the directors and officers of the as well the Directors and Officers of its member clubs, for their exposure to legal action arising from actual or alleged wrongful acts while performing their duties as officials of the organization. Sport Accident Insurance This policy covers members of who has sustained an injury while participating in a sanctioned activity. This policy will cover for accidental Death and Dismemberment as well as medical bills for a member or, who has sustained an injury while participation in a sanctioned activity. This policy is secondary to any other health care plan.

IMPORTANT: This "Summary of Coverage" is intended for use as evidence that the insurance coverage described herein is in force. It is subject to the standard terms, definitions and conditions of the policy issued by the insurer(s) for this type of insurance, the conditions, and limitations and replaced by the actual policy, whichever occurs first. The insurer(s) agree to notify Dan Lawrie Insurance Brokers Ltd. in writing, if the policies issued do not conform to the coverages outlined in this Summary of Coverage. CANCELLATION CLAUSE: It is hereby understood and agreed that the Cancellation Clause of this insurance is amended as indicated above within this "Summary of Coverage". In the event the insurer(s) initiate cancellation, they must do so in writing, by Registered Letter stating the above noted notice period, with the exception of cancellation for non-payment when the Statutory Conditions of the policy(ies) shall apply. 6 Important Terms Participant Liability This coverage protects and its members from claims arising from bodily Injury and Property Damage in the event an injured athletic participant files a lawsuit. It also protects the participant in the event that one player is sued by another player as a result of an injury. Most General Liability policies excludes Sports Participants Liability Tenants Legal Liability- Covers legal liability for damage to property leased to or rented by the Insured where the damage is caused by the Insured's operations and the Insured is held legally liable. Non-Owned Automobile Coverage- Indemnifies the insured for the legal liability arising out of the operation of non-owned vehicles used in the Insured's business (e.g. an employee using his own car on a business errand). It is a general rule of law that an employer is responsible for the negligent acts or omissions of an employee in the course of employment and hence could be involved in claims arising from the employee's negligent operation of the employee's own vehicle. Incidental Medical Malpractice- Protects you against claims for injury or sickness arising from your arrangements to provide treatment and/or facilities (e.g., the operation of first aid facilities). This coverage is not intended to replace any Professional Malpractice insurance that may be carried by any medical or paramedical practitioners employed by you. Directors' and Officers' Liability -Covers directors and officers of corporations and institutions for the personal liability they incur based on some allegation of negligence, error or omission and defined as a "wrongful act" in managing corporate affairs and where the injured party has suffered loss. Payment under the policy may be made directly to directors and officers for their personal liability or to corporations when they reimburse directors and officers for liability incurred. Exposure to directors and officer s liability applies to large conglomerates, smaller operations and non-profit organizations including entities such as condominium corporations. Cross Liability Clause- This clause allows for additional insured s to sue, if necessary within the policy. Contractual Liability-this provides coverage for the insured when he/she signs a contract, which stipulates the legal responsibility of the insured.

7 CLAIMS PROCEDURES Liability Claims: All Liability Claims, Notice of Claims, Notice of Potential Legal action, must be reported to Ontario Lawn Bowls Association who in turn will submit them to Dan Lawrie Insurance Brokers. Accident Claims Instructions: Everest Insurance Company of Canada must receive your completed claim forms within thirty (30) days of the accident occurring. Complete the attached Sport Accident Claims Form (found on OLBA website) and have your physician complete the Attending Physician Statement. If your claim is for dental injury have your dentist complete the Attending Physician Statement. Forward original forms along with copies of expense receipts and statements of reimbursements from your personal insurers to: Laurie Hawdon Claims Department 130 Bloor Street W. 8 Request for Certificate of Insurance A member club of the may require certificate of insurance adding an additional insured. One can be obtained by ing Nadia at: olbacertificates@danlawrie.com Please be sure to include the following information in your Your club name and address Name of the additional insured including the full mailing address Dates of the use of Facility- If applicable. If certificate is required by a contract, please forward copy of contract for our reference.

9 OLBA Clubs Commercial Property Coverage Dan Lawrie Insurance Brokers is pleased to provide the (OLBA) an optional Club Property Policy for that will provide all member clubs with a very low-cost insurance solution for their club property and content coverage. Highlights of coverage: Property of Every Description : All Risks Property Wordings EDP Equipment Data & Media $50,000 Flood - $10,000 deductible Money $ Securities (Destruction/Disappearance) $10,000 Replacement Cost on contents Sewer backup- Full Property Limits Earthquake- $50,000 Deductible Extra Expense-$25,000 Eligibility Policy Period: Proof of Coverage: Cost: This would be a Master Policy that will cover all OLBA member clubs for the coverages outlined. Any club requiring more coverage will need to contact Dan Lawrie Insurance for information. January 1, 2017 January 1, following the underwriting and acceptance of a completed application and payment in full to DLIB office. The Master Policy is issued to the OLBA office.
