5 LIFE INSURANCE CORPORATION OF INDIA INSTRUCTIONS IN REGARD TO SUBMISSION OF EMPANELMENT APPLICATION NOTE: The contract should note that non-compliance of the following instructions will render the application liable f rejection. 1. The envelope containing documents should be super scribed Empanelment of Contracts/ Agencies f Civil wks and Modernization Wks including Electrical installation, Air Conditioning and Fire sprinkler system in Tamil Nadu, Pondicherry & Kerala state. 2. Address to which the filled in Enrolment Fm is to be submitted: The Chief Engineer, L.I.C. of India, Southern Zonal Office, 153, Anna Salai, Chennai Last date f receipt of applications: up to hours. 4. The contract has to pay Rs.2,000/- ( Rupees Two Thousand only ) as a Processing fee FOR EACH APPLICATION ( Non refundable )in the fm of Demand Draft/Pay Order/Banker s Cheque in favour of Life Insurance Cpation of India payable at Chennai. 5. If the contract/ agency is eligible f Civil wks and Modernisation/Interi Wks, has to submit separate enrolment fm and separate processing fee f each Nature of wk. The Enrolment Fm along with the Annexure A1, A2 and B to G shall be downloaded in excel fmat and completely filled by either typewriting hand writing in all respect along with the cost of processing fees of Enrolment Fm of Rs 2000/-(non refundable) by way of Demand Draft/Pay Order/Banker s Cheque in favour of Life Insurance Cpation of India Please note that no consideration will be given f postal delay. Contracts to note that all particulars required as per the fm and Annexures shall be filled in completely in relevant strictly as per the fmat. The fms not submitted strictly as per the above instructions within stipulated period are liable to be rejected. 5

6 The empanelment of the eligible agencies, which fulfill the selection criteria, shall be evaluated. Please note that no enquiries crespondence regarding the selection f issue of tenders shall be entertained. Latest Solvency Certificate from any Nationalized/Scheduled Bank of minimum amount as mentioned in the Eligibility Criteria as per advertisement should be submitted along with application. Full address of the site of wk, owner authity under whom the wks have been carried out should be given (Please refer Annexure D & E). The contract/ agency should ensure to submit the satisfacty Completion Certificate giving the value of wk, year of completion and it should also tally with the value of final bill in Annexure D. The annual turnover should be based on latest Income Tax Clearance Certificate duly cleared by Income Tax Department audited balance sheet, copy of which should be enclosed. All the enclosed documents are to be self attested by the authized signaty Copy of Agreement in case of wks carried out f private agencies should be enclosed. Contracts/Agencies shall be categized based on the Eligibility Criteria solemnly by the discretion of Chief Engineer. Photo copies of self attested PAN, GST registration certificates are compulsily enclosed along with the enrolment fm. In the absence of above, the enrolment fm shall not be considered. 7 CONTRACTOR
