1 Oak Lawn/Worth Investments 16W571 Mockingbird Lane #101 Willowbrook, IL / Fax Criteria For An Application The following are eligibility requirements for consideration in approving an applicant for tenancy. 1. The application must be fully completed, signed and initialed by each person who is 18 years of age or older. 2. Each person who is 18 years of age or older and will be residing in the unit, even if they will not be contributing financially, must complete the application and submit a non-refundable application fee of $25.00, payable by cashier's check or money order only. All applicants must submit a photo ID and the most recent two pay stubs for the Management Office to copy. If the office is closed original pay stubs must be submitted and will be returned upon request after copies are made. 3. Leaseholders must net at least double the monthly rent to qualify. 4. Management will consider whether the applicant has had a satisfactory history of meeting financial obligations, including timely payment of rent, outstanding jugement or history of late payments of bills. Co-signers will not be allowed with the exception of full time students. 5. History along with previous landlord's recommendations must be approved. Management will consider whether the applicant or any person living in the apartment has a history of disturbing the neighbors, destroying property or has exhibited living habits at a prior residence which could adversely affect the health, safety or quiet enjoyment of other residents. 6. The unit applied for must have enough bedroom space to accommodate the applicant's household. Management will consider 2 people per bedroom to be the maximum number of persons a unit will hold. 7. Management will automatically deny any applicant or any person who will live in the unit and who has been convicted of a felony or is a sex offender, has filed bankruptcy in the past 3 years or who has had an eviction filed against them in the past 5 years. 8. Security deposit amount is determined based on credit; minimum security deposit amount is equal to one month market rent. An additional non refundable pet fee is required if applicant has or acquires pets during their occupancy. There is a $ pet fee required for one pet and an additional $ required for a second pet. NO TWO DOGS are allowed. 9. Utilities: Based on location 10. Laundry: Coin operated laundry facilities are located in each building. 11. Proof of renters insurance must be provided at move in and maintained throughout tenancy.
