1 Financial Assistance of ipod Touch Application Procedure 1. Please read the Eligibility Criteria and the Terms and Conditions. 2. Attach the following supporting documents to the Application Form: a. Photocopy of Identification Documents for All Household Members: (i) (ii) (iii) (iv) Applicant s NRIC and Student ID Card NRIC for all family members 16 years old and above Birth Certificate for family members below 16 years old Student Cards for siblings who are full time student b. Photocopy of Income Documents for All Working Family Members: (i) (ii) (iii) (iv) Past 3 months salary slips or letter from employer indicating the fixed gross monthly income OR Past 6 months CPF contribution history and Income declaration form declaring the gross monthly income OR Notice of Assessment from IRAS and Income declaration form declaring the gross monthly income Any other documents to support your application 3 Submit the Application Form, together with and all required supporting documents to 4 Student & Alumni Services Office at BLOCK 22, LEVEL 3. 2 ELIGIBILITY CRITERIA APPLICATION OF FINANCIAL ASSISTANCE FOR IPOD TOUCH INTEREST-FREE LOAN For Interest-free Loan Scheme of up to $300 for ipod Touch For students enrolled in Full-Time Diploma in Health Sciences (Nursing) [HSN] Course. Your gross household income shall not exceed $2,000 per month. This is the total gross monthly income of applicant's parents, siblings and other family members staying in the same household. It includes Basic Salary, CPF, Allowances and Commission. If you need assistance to purchase an ipod Touch but do not meet the eligibility criteria, please submit your application and state the reason(s) in the on-line application form. 3 TERMS AND CONDITIONS FINANCIAL ASSISTANCE FOR IPOD TOUCH INTEREST-FREE LOAN Frequency This financial assistance scheme is available to students only once during their course of study in Ngee Ann Polytechnic. Ngee Ann ipod Touch Suppliers The financial assistance is only available for purchase of ipod Touch from the Ngee Ann Polytechnic's approved suppliers. Use of ipod Touch The ipod Touch shall be used by student for his/her course of study in Ngee Ann Polytechnic. He/she shall not lend, hire or sell the ipod Touch to any other person. The Polytechnic reserves the right to withdraw the interest-free loan if student is deemed to be using the ipod Touch for purposes other than for his/her study. Loan Quantum The loan amount shall not exceed $300 (inclusive of GST) or the purchase price of the ipod Touch, whichever is lower. Student is allowed to top up the loan amount for an ipod Touch that costs more than $300 (inclusive of GST). * If you have taken up an interest-free loan for another mobile computing device required by the course, the interest-free loan amount for the ipod Touch shall be the balance of $2,000 less the interest-free loan already taken up for the other mobile computing device or up to $300, whichever is lower. Repayment Loan repayment shall be billed together with the fee voucher on a semester basis and deducted through Interbank GIRO. Students are required to pay $50 per semester, with all outstanding balance of the loan payable in the final semester. In the event that a student leaves the Polytechnic without completing his/her course of study, the loan shall become due and payable immediately. Page 3 of 5
