We present this ANZ Credit Card to you, who wish to obtain a variety of benefits, facilities and ease of transaction. As an exclusive member, you are entitled to freedom and flexibility in shopping with a high credit limit. This guidebook contains an outline of information you require in order to be able to enjoy all special services and benefits your ANZ Credit Card has to offer, and is integral and inseparable part with credit card flyer and credit card application form. IMPORTANT INFORMATION You acknowledge that as part of the transfer of our Retail and Wealth Management Business of PT Bank ANZ Indonesia ( ANZ ) to PT Bank DBS Indonesia ( DBS ) that we expect to be completed by February 2018 or a later date that you will be advised of, your banking products and services that you have in place with ANZ will be transferred to DBS. ANZ Card Centre Your ANZ Credit Card limit is indicated on the card delivery sheet and monthly billing statement. The amount you can use for ordinary shopping or fund transfer service is the amount of Credit Limit or the amount of remaining credit available in your card. In order to ensure that your Credit Card can be used at all times, it is recommended that you conduct transactions which do not exceed your Credit Limit. In an emergency situation or while you are out of town or overseas, you may request temporary increase of Credit Limit by contacting the 24-Hour ANZ Call Centre.

4 3. YOUR CARD Your Name Ensure that your name is printed correctly in accordance with your request. This ANZ Credit Card cannot be transferred to another person. 2. Your ANZ Credit Card Number This is the 16-digit number of your ANZ Credit Card. Please indicate this card number for every bill payment and your correspondence. 3. Validity Period The month and year indicated next to Member Since and below Valid From and Valid Thru indicate the validity period of your ANZ Credit Card. The card validity period will be automatically extended as long as there is no notification of card closure. Your new card will be delivered one month prior to the expiry of the old one. 4. MasterCard/Visa Logo The MasterCard/Visa logo on your ANZ Credit Card guarantees that your ANZ Credit Card is accepted at all places throughout the world displaying this logo. 5. Chip The Europay MasterCard Visa Chip technology is a state-of-the-art standard recognized by MasterCard and Visa International for security and comfort in conducting transactions. 5 6. Magnetic Tape This Magnetic Tape stores data used for acting upon requests for authorization of your transactions or for ATM cash withdrawal transactions. Keep your card away from magnetic objects (electronic equipment) as it may result in data on the magnetic tape being erased. 7. Signature Column The signature column is the place to put your signature as the Holder of this ANZ Credit Card. Sign your ANZ Credit Card immediately in this column in order to prevent other people from misusing your ANZ Credit Card. 4. YOUR EUROPAY MASTERCARD VISA (EMV) CHIP TECHNOLOGY We have equipped this ANZ Credit Card you receive with the Europay MasterCard Visa (EMV) Chip technology enabling you to conduct transactions in a more secure and comfortable manner. Ensure that you receive your ANZ Credit Card back after the transaction process has been completed. Activation and Setting Up a PIN As indicated on the card safety sticker, in order to be able to conduct retail transactions and to withdraw cash, you need to do the following: - for a new card, you are obligated to activate the card and to set up a PIN. - for an extension or replacement card, you are obligated to set up a PIN. Steps to activate your ANZ Credit Card: 1. Through ANZ Internet Banking Access and follow the subsequent instructions. 2. Contact 24-Hour ANZ Call Centre Press 112 and follow the subsequent instructions. After your new card has been activated, remove the sticker on the card and proceed immediately to the process of setting up a PIN so that the card can be used for retail and cash withdrawal transactions. 6 7 You need to pay attention to the following several important matters in using ANZ Credit Card with chip technology: While using it, your ANZ Credit Card is to be inserted (rather than swiped) into the available terminal slot. The magnetic tape which is also on the back of your ANZ Credit Card can still be swiped as usual to complete transactions, if the available terminal is unable to process transactions using a Credit Card with chip or if a transaction using a Credit Card with chip is unsuccessful. 5. PIN A PIN must be used when you conduct retail and cash withdrawal transactions in-country and overseas using ANZ Credit Card. You may set up/change a PIN in 2 ways: 1. Through ANZ Website. Access and follow the subsequent instructions.

Through 24-Hour ANZ Call Centre by taking the following steps: - Send an SMS to 3269 in order to obtain Authentication Code SMS format: PINCC<space>date of birth of the main cardholder (ddmmyy)#last 4 digits of the credit card number. Example: PINCC # Contact or (021) Press 2111 to go directly to the PIN menu. - Press 1 to set up a new PIN or Press 2 to change a PIN - Enter the Authentication Code and follow the subsequent instructions. The PIN of an additional cardholder must be set up by the main cardholder with the authentication code to be sent to the main cardholder s Cellular phone number registered in the ANZ system. Contact 24-Hour ANZ Call Centre to update your Cellular phone number. In order to maintain confidentiality and security, memorize your PIN and never disclose your PIN to other parties and never write a PIN together with the Credit Card. SUPPLEMENTARY CARD Every Basic Card may hold up to 3 (three) Supplementary Cards. Each Supplementary Card has a different number from the Basic Card with a combined credit limit. To help you sort transactions, control card use, and make payments for the bills of each card, as the Basic Cardholder, you will receive details of the use of Supplementary Card along with the Basic Card. If you wish to give a Supplementary Card to your family member or relative, please contact 24-Hour ANZ Call Centre. 9. CASH WITHDRAWAL FACILITY By using a PIN, you can easily withdraw cash at all ATMs bearing MasterCard/Cirrus and Visa/Plus logos throughout the world. The total amount of cash withdrawal will be indicated in your monthly billing statement. Cash can be withdrawn at: ATMs bearing MasterCard/Cirrus and Visa/Plus logos throughout the world All branch offices of banks issuing MasterCard/Visa cards in Indonesia ATMs overseas For each ATM cash withdrawal overseas, the ATM s Bank will charge an additional fee ( ATM Fee or Terminal Fee ) in the amount to be determined by the ATM s Bank. 10. INTEREST If the total amount of bill is not fully paid or effective payment is received following the Due Date or if no payment is made at all, you will be charged with interest which will be calculated based on the applicable interest rate on each type of transaction as from the posting date up to and including the date of full payment, calculated on a daily balance basis and to be adjusted to payments made by the Cardholder and received by the Bank. ANZ shall not include outstanding interest, fee and penalty as components in calculating credit card interest. Interest will be calculated as follows: 10 11

8 a. Retail Interest Calculation Retail transaction interest shall be calculated as from the posting date. Retail transaction interest shall be charged if no payment is received by ANZ on the payment due date indicated in the billing statement or if no payment is made or payment is not made in full. Expenditure transaction interest for the subsequent bill shall be calculated on the remaining bill with regard to outstanding expenditure transactions. b. Cash Withdrawal Interest Calculation Interest shall be calculated and charged as from the posting date of a cash withdrawal transaction up to and including the date of full payment, on the amount of cash withdrawal plus interest. 9 12. ANZ GUARD ANZ Guard protects you from the risk of inability to pay your ANZ Credit Card bill in the event of demise or temporary/permanent disability. ANZ SPOT ANZ has cooperation networks with various leading restaurants and hotels allowing you to enjoy various benefits and discounts by using ANZ Credit Card at places bearing ANZ Spot logo. 15. MISCELLANEOUS All reference to the name ANZ in this guidebook is intended to refer to the legal name of PT Bank ANZ Indonesia. Every information in this guidebook is correct at the time of issuance but it may be subject to change at any time upon prior notification. 10 CARDHOLDER AGREEMENT 1. INTRODUCTION Before you use ANZ Credit Card, it is extremely important for you to read and understand these Terms of Usage. For any purpose, this Cardholder Agreement will be considered as an agreement between you and the Bank and you hereby acknowledge that by using the card, you accept and shall be unconditionally bound by the Terms of Usage stipulated in this document and that your act of using the card shall mean that you officially and validly accept this agreement. In the event that you are unable to accept these Terms of Usage (or any part or parts thereof), you must contact ANZ Card Centre in writing and/or verbally within 2 business days upon receipt of card. In the event that you do not contact the bank within 2 business days, it is deemed that you will be using the card. The Terms of Usage shall apply up to the closing of Card account as provided for under these Terms of Usage. Any amendment thereto from time to time shall be notified in advance by the Bank. 2. DEFINITIONS The following definitions shall apply to the entire Terms of Usage, unless specifically stipulated otherwise. 1. Terms of Usage or Cardholder Agreement shall be the contractual framework by which the Bank and the Cardholder shall act and for any purpose, shall be deemed as an agreement between the Bank and the Cardholder. 2. Bank or ANZ refers to PT Bank ANZ Indonesia. 3. Card shall be every Credit Card issued by the Bank for you or for every additional Cardholder for operating a Card account. 4. Basic Card shall be a Credit Card issued by the Bank for the main Cardholder 5. Supplementary Card shall be a Credit Card issued by the Bank for the additional Cardholder (as defined below). 6. Cardholder or You shall be any person received a card for operating the designated account with the Bank, including the Account Holder. 7. Basic Cardholder shall be a Cardholder with whom the Bank enters into a main or principal agreement in relation to the use of card and Card account. 8. Supplementary Cardholder shall be a person you appointed, for whom the Bank has issued a card to be operated in your Card account. 9. Card Account shall be ANZ Credit Card account accessed by your card. 10. Account Holder shall be a person whose name is used by the Bank for opening a Card account and who shall be fully responsible for all Transactions and obligations arising with regard to the Card account. 11. Transaction, Fee or Card Transaction shall be all financial Transactions with respect to a Card account. 12. Merchant shall be any store/company/another party receiving your card for the purchase of items and/or services and/or cash withdrawal. 13. PIN shall be a Personal Identification Number, substitute for signature for all retail and cash withdrawal transactions in-country and overseas using ANZ Credit Card. The current number of digits of ANZ PIN is 6 digits. 14. Billing Date shall be the date of the last day of billing cycle at which the card Account Billing Statement indicates and 16 17

11 includes all transactions received and processed by the Bank up to the end of working hours on such date. Due Date shall be the date on which your payment must be received by the Bank s Card Centre to be processed by the Bank (you need to wait for at least two business days as from the date on which you make payment at your bank until your payment is indicated in your account). Outstanding Balance or Closing Balance shall be the amount you must pay for your Card account on a certain day and which will be billed to you in the monthly Billing Statement. Minimum Amount Due shall be the minimum amount you must pay for your account to meet these Terms of Use. Overdue Amount shall be the Minimum Amount Due (if any) which remains outstanding from your previous Billing Statement. Immediate payment must be made for the total Overdue Amount. 24-Hour ANZ Call Centre shall be the facility provided by the Bank to a Cardholder via telephone by which a Cardholder may obtain various types of information or file complaints concerning the card by contacting certain telephone numbers designated by the Bank for such purpose whereby the Bank may from time to time change the information and telephone numbers provided. Cardholder Agreement. Therefore, by using the Card, it shall be deemed that you have no objection to, that you accept and submit to all terms and conditions of the Cardholder Agreement. 4. OWNERSHIP The card shall remain the property of the Bank at all times and it shall be provided to the Cardholder for use in accordance with the Terms set out in the Cardholder Agreement. Card usage shall be limited to the Cardholder and it shall be subject to other conditions stipulated in the Cardholder Agreement, and the Card shall remain valid up to the expiry date indicated on the card. The Cardholder shall not be allowed to give the Card to another party and/or allow another person to use the Card and shall make all endeavors at all times to protect the Card. The Cardholder shall not be allowed to inform any other person, other than the Bank, about any personal information related to the Card Account, including but not limited to the issued PIN, and shall procure that the Card be under his/her personal supervision at all times. 5. DISCLOSURE AND SUBMISSION OF INFORMATION AND DOCUMENTS The Cardholder agrees to provide the Bank with all information and documents it may require from time to time, including information and documents which may be required under the law and/or agreement with the relevant (local or overseas) authorities concerning the tax status of the Cardholder ACCEPTANCE OF CARDHOLDER AGREEMENT A Cardholder s acceptance of the terms and conditions of the Cardholder Agreement shall be merely based on the use of Card by the Cardholder concerned as stated in Article 1 of the The Cardholder agrees and acknowledges that the Bank shall be entitled to disclose any information concerning the Cardholder (including, without limitation, all forms of transaction, financial condition account and services) to any or all of the following parties:

12 a) b) c) All institutions appointed by the government related to law enforcement Association of Credit Card issuers and Credit Card companies or other financial institutions Any party, when the Bank is required to do so in accordance with the law in the relevant jurisdiction and/or transparency obligation of the Bank based on an agreement with the relevant local or overseas authorities, whether such transparency is applied directly or through a Group member bank. The Bank shall not disclose any information concerning a Cardholder s Card Account to any third party other than the aforementioned parties, unless the Bank deems it necessary for its interest and/or in the event that the Bank is required to disclose information concerning a Cardholder s Card Account by applicable laws and regulations in Indonesia and by an authorized official of the Government of the Republic of Indonesia. The request for such information from an authorized official pursuant to the law shall be immediately fulfilled by the Bank. The Cardholder and any Supplementary Cardholder shall give approval to the Bank to inform about data processing including transactions and information concerning the Cardholder and Supplementary Cardholders. In relation to disclosure by the Bank based on this paragraph, the Cardholder agrees not to involve the Bank with respect to any consequences arising from disclosure of information concerning the Cardholder and/or Supplementary Cardholder to any third party. 6. CREDIT LIMIT The credit limit for your Card Account shall constitute the maximum amount of transaction you may conduct and which will be provided at the Bank s absolute consideration, with due observance of the applicable banking provisions. The Bank shall be entitled to increase or decrease at any time the credit limit (with or without your prior request) to the extent that it does not violate the applicable provisions of law. The Bank shall notify the cardholder of any change in credit limit. The Credit Limit may be unconditionally cancelled at any time by the Bank or automatically cancelled by the Bank in the event that the debtor s condition declines to sub-standard, doubtful, or bad. The Bank shall also be entitled to stipulate separate limits for cash and non-cash transactions (within the combined credit limit of your Card Account) and shall be entitled to determine a certain limit for the Main and Supplementary Cards. The Bank shall notify the Cardholder of any change in credit limit. The Bank shall be entitled to accept or reject transactions with the Card the use of which is about to exceed or has exceeded the stipulated Credit Limit. In the event that the Credit Limit which will be/has been provided by the Bank equals or exceeds a certain limit in accordance with the applicable regulations, for the Credit Limit to be effective, the Cardholder shall be obligated to submit a photocopy of the most recent Taxpayer Identification Number The Cardholder agrees to validate/acknowledge /approve the records/copies/microfilm prepared by the Bank as valid and binding evidence with the same legal force as the original. The Cardholder agrees to provide additional information and documents requested by the Bank from time to time. 7. AVAILABLE CREDIT Available credit in your Card Account shall be the unused value of the credit limit within a certain period. The available credit shall be calculated by considering credit limit, Outstanding Balance in your Card Account and Fee or Transaction which has been received by

13 the Bank but not yet received for processing on such date. With due observance of the applicable banking provisions, the Bank may stipulate a separate credit limit for cash and non-cash Transactions and shall inform about such limits from time to time. In the event that separate limits are stipulated, the available credit for cash withdrawal Transactions will be determined from time to time by the cash withdrawal credit limit less each (outstanding) cash withdrawal Transaction and each authorization of cash withdrawal conducted, but not yet posted. However, for non-cash Transactions, the available credit at any time shall be determined by the total amount of credit limit less the amount of outstanding (cash and non-cash) Transactions and each authorization of Transaction (cash and non-cash) conducted, but not yet posted. You must take responsibility for conducting Transactions in your Card Account by keeping the Card use within the available credit limit (cash and non-cash, as applicable) of your Card Account at all times. The Bank shall also charge a fee each time the Card Account balance exceeds the credit limit at the value determined by the Bank and notified to you from time to time. 8. DAILY CASH WITHDRAWAL LIMIT The Bank may limit cash withdrawal from your Card Account by stipulating a daily cash withdrawal limit the amount of which shall be notified in advance to you. This amount shall constitute the maximum cash withdrawal value which you are entitled to withdraw each day, based on the available credit in your Card Account (as explained in Article 7 above). has married based on the Basic Cardholder s application. The Basic Cardholder shall be bound by the Card use by Supplementary Cardholder(s) and shall be responsible for every bill, fee and expense arising from the use of Supplementary Card and the Bank shall be entitled to collect all such fees through your Card Account. The Bank shall be entitled to cancel any or all Supplementary Cards issued to you by notice to the Basic Cardholder. In addition, upon the Basic Cardholder s request, the Bank shall cancel all Supplementary Cards. However, all Transactions which have been conducted with respect to the Supplementary Card shall be deemed applicable and binding on the Basic Cardholder until all bills of the Card have been fully paid. Furthermore, the Bank shall not repay any fee or charge which has been charged to your Card Account. 10. VALIDITY PERIOD, TIME LIMIT AND CARD RENEWAL A card shall be valid for the validity period as indicated on the front of each Card. The expiry of validity period of a Supplementary Card shall be the same as the expiry of validity period of the Basic Card regardless of the time at which the Card is issued. The Card s validity period shall expire on the last day of the month of the time limit indicated on the front of your Card (the Card may be cancelled by closing or cancellation earlier than this date as explained further in the Cardholder Agreement). Upon receiving the Card, the Cardholder can obtain a PIN by following the steps set out in the PIN section SUPPLEMENTARY CARDS Upon the Basic Cardholder s request, the Bank may issue a Supplementary Card to any person who has reached the age above 17 years or who Unless a Card Account or Cardholder violates a Term stipulated in the Cardholder Agreement, the Bank shall automatically renew the validity

14 period of your Card and send a new Card, in general within a period of one month prior to the expiry of the Card s validity period. Under certain circumstances, the Bank may issue a replacement Card for the Card you hold with a new Card number. In the event that the number of the new Card or replacement Card issued is not identical to the number of the previous Card, the Cardholder must re-obtain a PIN by following the steps set out in the PIN section. In the event that the Cardholder does not receive a replacement Card or renewed Card within the stipulated time, please contact 24-Hour ANZ Call Centre immediately. In the event that the Cardholder does not wish to renew the Card s validity period, the Cardholder must notify the Bank by no later than 45 days prior to the expiry of the Card s validity period. If you do not do the above, the Bank shall continue to renew all Cards related to your Card Account and charge the applicable fees which you must pay. 11. ACCEPTANCE A Card shall be accepted at a Merchant if the Card sets out the signature of the Cardholder concerned on the back of the Card. A Card shall be accepted at any Merchant bearing the relevant Card logo (MasterCard or Visa). However, the Bank shall not be responsible for or the Bank shall have no obligation to the extent that the Merchant s rejection for any reason and for any act or loss resulting in inconvenience to you is not caused by manifest and wilful negligence by the Bank. The promotional material of the Card or the Card logo displayed anywhere shall not guarantee you the ability to purchase available items and services at such place by using your Card. However, you may report to the Bank any rejection/act/loss by a Merchant by specifying the Merchant s name, date and time of occurrence and all information which will assist the Bank in its investigation. 12. CASH WITHDRAWAL With the Bank s authorization, you may withdraw cash by using your Card, up to the amount equal to the available (cash) credit limit at any ATM bearing MasterCard/Cirrus and/or Visa/Plus logos and at any of the Bank s counters which issues or accepts a MasterCard and/or Visa Credit Card. The Bank may stipulate the maximum limit for daily cash withdrawal, which may be subject to change at any time and may revoke or change such facility with prior notice to the Cardholder. Cash withdrawal shall incur additional Transaction fees in accordance with regulations applicable from time to time. Banks providing cash withdrawal using your Card via ATM may stipulate the minimum and/or maximum Transaction limit you must know and understand before conducting a Transaction. 13. BILLING STATEMENT The Bank will manage the Account in the name of the Basic Cardholder in relation to the Card Account in which all Transactions, any other obligations of the Cardholder arising under these Terms of Use and any losses suffered by the Bank as a result of the use of Card or Card number, shall be charged. The Bank will deliver a Billing Statement detailing transactions charged to your Card Account, every month on the previously determined date. The Billing Statement shall be delivered to the Cardholder in the form of e-statement to the address specified by the Cardholder in the ANZ Credit Card Application or to the most recent address known to the Bank. Specifically for a Billing Statement delivered to the billing address, each Billing Statement shall be deemed received by the Cardholder within 12 days following delivery by the Bank. The Bank will include all Transactions related to the supplementary cards in the combined billing statement for your Card 24 25

15 Account. If no Outstanding Balance and no Transaction has been received by the Bank as from your most recent billing statement, the subsequent billing statement shall not be issued. If a billing statement is not received for any reason, it shall not constitute a valid reason for not making payment for at least the Minimum Amount Due obligation in your Account. In the event that you do not receive a billing statement within of 12 days as from your Billing Date, you shall be responsible for contacting the Bank in order to determine the amount you must pay and you must immediately make such payment. You may request a copy of the billing statement from the Bank, for which you shall be charged by the Bank. If you will not be present at the billing address you have designated at the time you usually receive your billing statement, you must make arrangement for another alternative to make payment for your Card Account. If there is a bill which does not match your billing statement, the Cardholder shall be obligated to contact the Bank to file an objection by no later than 30 days as from the Billing Date. 14. CARD USAGE The Cardholder shall be responsible for the usage of all facilities provided by the Bank in relation to the Card Account and for all relevant fees. All Transactions (in addition to Transactions order by mail, telephone, internet, and ATM cash withdrawal Transactions) shall require a PIN at the time of transaction or signature of the Cardholder on the Transaction slip, each time the Card is used by the Cardholder. A copy of the Transaction slip provided to you must be kept at least until the Transaction is presented on the billing statement. By entering a PIN or signing the Transaction slip, you shall be responsible for the value, receipt of items, services or cash (as the case may be), including any fee, or service bill indicated in the document. The Cardholder shall also be fully responsible for all additional Transactions which will arise from the Merchant according to the knowledge of the Cardholder. You may use Your Card to purchase or pay items and/or services offered through mail order, telephone, or online at internet sites by informing Your Card number to the Merchant. The transactions above need verification and authentication in the form of Cardholder Verification Value (CVV) and/or authorization code. By entering the CVV and/or authorization code then it is sufficient proof that ANZ has been given instructions to process the transactions. Hence, you are responsible to pay the bill of the purchase or payment transaction of the items and/or services. The authorization code is a feature of the 3D Secure in your card that enables you to conduct transactions in a more secure way. For types of transactions that require an authorization code to be sent to your cellular phone number, ANZ does not have an obligation to investigate the validity nor the authorize use of cellular phone number. To ensure the protection of the transactions, please make sure the cellular phone number you registered in ANZ system is correct and thine and immediately notify ANZ if Your SIM Card or mobile phone device is lost, stolen, damaged or transferred to other person

16 Transactions charged to your Account shall be deemed valid and authorized by you unless you dispute them in accordance with these Terms of Use. Copy of each transaction slip (for transactions indicated in you billing statement) may be requested by the Main Cardholder and the Bank shall provide it upon its absolute decision with additional charge. For each transaction using signature, in the event that the merchant fails to obtain the signature of the Cardholder on each slip of sale, voucher, cash withdrawal slip or coupon by mail order, the Cardholder shall not be released from his/her responsibility with respect to such slips or coupons. If you wish to cancel a transaction in your Card Account before it is completed, you must notify the Merchant accordingly and ensure that the Transaction slip is destroyed in your presence. Such being the case, (and in cases where after completing the transaction, whether using a PIN or signature on the Transaction slip, the Merchant agrees to cancel a Transaction on your Card) you must ensure that the Merchant concerned completes the refund Transaction process and provides you with a copy of the relevant Transaction slip which accurately records the information of your Card in such document (for instance, credit slip). You must keep the document and notify the Bank if the refund payment is not processed in your Card Account within 30 days following the date of refund payment Transaction. Credit Card is only valid for its purpose of retail transaction payment tool at merchants and cash withdrawal at ATM. Credit Card is strictly prohibited to be used for cash withdrawal at merchants or for any illegal purpose that against the law, including but not limited to purchase of good and services prohibited by laws and regulations applicable in the Republic of Indonesia. The Cardholder shall hold the Bank harmless against all liabilities (including, but not limited to, legal and financial liabilities) to the extent that they do not result from the Bank s manifest and wilful negligence if the card is used for purposes which violate the law or for illegal Transactions. In the event that the Cardholder files a protest in view of a Transaction not conducted by him/her, the Bank shall be obligated to conduct further investigation on such protest. The Cardholder shall be obligated to file a written protest (affidavit statement) and if required, the relevant Card for further substantiation. In any Transaction resulting from Card forgery (particularly card counterfeit or reproduction) confirmed by the Bank, the Cardholder shall be held harmless against any obligation to make payment for such Transaction and it shall be charged to/shall be borne by the Bank. 15. CHANGES IN PERSONAL DATA AND OCCUPATION The Basic Cardholder must notify the Bank as soon as possible, preferably in writing, of any change in personal data and occupation of the Cardholder, including but not limited to data related to identity number, home address, home telephone number, cellular phone number, address, occupation, office address, office telephone number and income per year. You also agree to immediately notify the Bank of any change in the name(s) of Cardholder(s). The Bank shall be entitled to accept your request pending valid evidence of the change. The Cardholder agrees to give clear instructions to the proxy appointed by him/her in Indonesia for settling his/her Card Account in the event that 28 29

17 the Cardholder wishes to leave Indonesia for a period of more than 30 days and or based on a Power of Attorney signed by the Cardholder. The Bank may at any time request you to update your newest data in accordance with the applicable provisions. 16. TRANSACTIONS OVERSEAS & IN A FOREIGN CURRENCY a. Credit Card Transactions in a foreign currency other than Rupiah shall be charged to the Cardholder s credit card account. Conversion shall be made first into United States Dollars and subsequently into Rupiah. In the event that a transaction is conducted in United States Dollars, it shall be directly converted into Rupiah. Conversion shall be made on the date on which the transaction is processed by International MasterCard or Visa, the processing date of which may be different from transaction date. The conversion exchange rate used shall be as determined by International MasterCard or Visa. The intended Transactions shall include credit card transactions in a foreign currency (other than Rupiah) at any merchant located within and outside Indonesia. All transactions in a foreign currency shall be charged with conversion fees by the Bank. b. The exchange rate along with conversion fees charged may be identified by dividing the rupiah value indicated in the bill by the value of foreign currency used during the transaction. The Cardholder shall be responsible for the transaction in a foreign currency other than Rupiah along with the foreign currency exchange rate risk. 17. TRANSACTIONS CONVERTED INTO INSTALLMENTS Any transaction converted into installments shall be subject to a conversion fee. The Reward Points of retail transactions in installments shall be effectively cancelled upon successful conversion of the transaction into installments (other than Kartu Cicilan & ipay). If an installment transaction is cancelled, the customer shall be charged with a cancellation fee applicable at the time of cancellation of the installment transaction and the Reward Points shall not be refunded. 18. ATM TRANSACTIONS The Cardholder may use a PIN at ATMs bearing the relevant Card logos (Cirrus, Maestro, Visa, PLUS and Visa Electron). You must take any action required to ensure the security of your Card and PIN. You should not write down a PIN on your Card or keep your PIN in a written note. You should not lose possession of your Card or disclose your PIN to any person. The Cardholder must assume full responsibility for all Transactions processed using Card at ATMs accepting the Card and hereby authorize the Bank to debit the Card Account with the withdrawal amount with or without the Cardholder s knowledge or special authorization. The Bank s record of the transaction processed by using Card at an ATM shall be conclusive and binding for every purpose. The Bank shall not be responsible for any loss or damage arising either directly/indirectly from malfunction/failure of the Card or ATM, temporary fund insufficiency at the ATM or any other matters arising from your fault in using the Card or ATM. The Cardholder shall not be entitled to make any withdrawal in excess of the credit limit for cash 30 31

18 withdrawal transaction. Administrative fee shall be charged by the Bank for each cash withdrawal to the Card Account of the Cardholder, with the value stipulated by the Bank and notified to the Cardholder from time to time. 19. LOST/STOLEN CARD The Cardholder must immediately notify the Bank upon realizing any loss or theft of Card or manifest disclosure or potential disclosure of PIN by contacting 24-Hour ANZ Call Centre and request the Bank to block the Card. The Notice must be immediately followed by delivery by the Cardholder to the Bank of (i) written confirmation made by the Cardholder, and (ii) copy of the police report confirming the occurrence of the aforementioned situation. The written confirmation and police report must be submitted to the Bank by no later than 2 x 24 hours following notification by the Cardholder. The Bank shall record such loss and take necessary actions to prevent misuse of the Card upon receipt of the notification. The Cardholder shall be responsible for any unauthorized Card Transaction (if any) up to the time at which the Bank receives the Cardholder s notification of the Card loss/theft and/or when the Bank receives written confirmation and police report, whichever occurs earlier. The Cardholder may deliver the written confirmation and police report to the Bank via facsimile. Such delivery shall be deemed to have been conducted or provided by the Cardholder when received completely and clearly by the Bank and delivered to the facsimile number determined or provided by 24-Hour ANZ Call Centre. Following such date, the Cardholder shall not have any further responsibility, provided that the Cardholder does not play any role in or does not cause the loss and does not commit any fraud or negligence related to the unauthorized Card Transaction resulting from the loss of theft of the Card or PIN disclosure. If the Cardholder eventually finds his/her Card, he/she must report the same to the Bank and may not attempt to use the Card and must immediately return the Card reported as having been lost or stolen to the Bank. Based on its absolute consideration, the Bank may approve or reject, upon your request, the issuance of a replacement Card for the lost or stolen Card, based on the same Terms of Use as the Terms of Use of the original Card as amended from time to time. The Bank shall be entitled to charge a fee for a replacement Card to the Cardholder s Card Account. Notwithstanding any other provisions of the Cardholder Agreement to the contrary, the Cardholder shall be responsible for all losses suffered by the Bank as a result of Card use by any person taking possession of the Card with the Cardholder s approval. 20. FEES AND CHARGES The Cardholder shall be responsible for and must pay the following: Fees and taxes incurred (which means taxes, user charges, charges, obligations, or tax deduction or withholding required to be made by ANZ in the payment to the Cardholder pursuant to the law or agreement with a foreign government or revenue authorities (both local and overseas). All fees with respect to the issuance and use of Card and any Supplementary Card. The fees constituting the Cardholder s obligation shall include, but shall not be limited to, the following: Basic Card annual fee Supplementary Card annual fee 32 33

19 Late payment fee Stamp duty fee Cash withdrawal fee Exceeded credit limit fee Replacement card fee Payment refund fee Billing Statement printing fee Transaction voucher or slip application fee Conversion of transaction into installment fee Installment cancellation fee Fund transfer fee Check/clearing account/auto-debit payment rejection fee Inter-bank transfer fee Payment rejection fee shall be fee charged to the Cardholder s Card Account if the payment made by the Cardholder by check/clearing account/auto-debit is rejected, in which case the Bank shall be entitled to charge the payment rejection fee to the Cardholder s Card Account. Annual fee shall be annual membership fee charged to the Cardholder s Card Account starting from the approval of the Cardholder s application by the Bank and shall be charged in the first Billing Statement. Supplementary Card annual fee shall be annual membership fee charged to the Cardholder s Card Account starting from the approval of the Supplementary Card application by the Bank, the amount of which shall be in accordance with the Main Card annual fee calculated on a pro rata basis as from the date of Bank s approval of the Supplementary Card application. In the event that an instruction, check or any other instrument of the Cardholder cannot be accepted, the Bank shall charge the Cardholder with a check/other instrument rejection fee with the value stipulated by the Bank and notified to the Cardholder from time to time. These fees and charges may be subject to change at any time with prior notice by the Bank. Details of all fees payable with regard to the Card can be obtained from the Bank if requested, and the Cardholder shall be responsible for obtaining and knowing the Bank s list of fees at any time. 21. INTEREST If the total bill is not fully paid or the effective payment is received following the Due Date or if no payment is made at all, the Bank shall charge interest which will be calculated based on the interest rate applicable for each type of transaction (expenditure or cash withdrawal and fund transfer transactions) as from the posting date up to and including the date of full payment, calculated on a daily balance basis and to be adjusted to the payment made by the Cardholder and received by the Bank. A. For Cash Withdrawal Interest shall be calculated based on cash interest rate as from the posting date of cash withdrawal Transaction up to the date of full payment of the amount of cash withdrawal plus interest thereon. Interest shall be calculated on a daily balance basis to be adjusted to partial payment (cash withdrawal fee shall be charged on such basis). B. For other transactions No interest shall be calculated in a Transaction fully paid within the due time limit of the report at which the Transaction is indicated for the first time. If no payment is made or partial payment is received up to and including the Due Date, the Outstanding balance shall be carried forward as posting balance in the subsequent report. If a report indicates outstanding and unpaid amount as opening balance, interest shall accrue on the opening balance as from the Report Date up to and including the date of full payment. In the event that the bill in the first month is not 34 35

20 fully paid and new transactions are conducted in the second month, such new transactions in the second month shall not be subject to interest if already paid in full prior to the due date. Interest shall be charged if the bill in the second month is not fully paid either. Interest shall be calculated on a daily balance basis and shall be adjusted for partial payment. The total interest shall accrue up to and including the Report Date plus the following non-business days (Saturday, Sunday and other Bank Holidays), which shall apply on the Report Date. 22. PAYMENT All payments to your Card Account must, unless specifically recommended otherwise by the Bank, be made in Rupiah currency. The Minimum Amount Due on your billing statement shall be due on the Due Date. You hereby acknowledge that a Due Date is merely a facilitating means to enable you to receive the billing statement and also to give you time to remit your payment and under any circumstances you shall not be entitled to delay or avoid payment if your billing statement is not received or delayed. If you do not intend to pay the full amount of the Closing Balance indicated in your billing statement, you must pay not less than the Minimum Amount Due indicated in your billing statement and payment shall be effectively received by ANZ by no later than the Due Date. Any amount overdue or exceeding the credit limit indicated in the billing statement must be paid immediately. Minimum Amount Due shall be the minimum amount payable by the Cardholder. The amount shall be calculated based on a certain percentage, notified by the Bank from time to time, of the Closing Balance less Fixed Installments plus a certain percentage, of the fixed installment, amount amount exceeding the credit limit and any amount overdue in your previous Billing Statement. With due observance of the applicable banking provisions, the Bank may determine the minimum amount of the Closing Balance in which the provision concerning the Minimum Amount Due shall not apply and the total Closing Balance must be paid. Prior to the closing, suspension, or cancellation of the Card, the minimum payment facility shall no longer apply and regardless of the Minimum Amount Due as indicated in your billing statement, you must pay the Closing Balance in full. In general, the payments received by the Bank shall be applied to your Card Account in order to pay various outstanding components in the following order: - Fee and Penalty on your Credit Card; and the remaining payment shall be allocated for: - Transaction Principal Value (60% of the remaining payment) - Interest (40% of the remaining payment) Any surplus payment (after paying the above components) shall be indicated as credit balance in your Credit Account. 23. DIRECT DEBIT FACILITY The Bank may enter into agreements with other banks in Indonesia, hence the Cardholder holding accounts at such other banks may provide fixed instructions to such other banks to directly debit the Minimum Amount Due or the Closing Balance of the Cardholder s Card Account to the Cardholder s savings accounts or current accounts at such other banks. In the event that the facility is provided, the Minimum Amount Due or the Closing Balance shall be 36 37
