Box 10 Berwick, NS B0P 1E0 TF: EO Identity Fraud Endorsement Insuring Agreement If the Declaration Page shows that the Identity Fraud Endorsement applies we agree that coverage is extended to include reimbursement of costs you actually incur resulting from Identity Fraud as defined and limited herein. The maximum limit paid in any one occurrence will be shown on the Declaration Page. Definitions You and your in this wording have the same meaning as in the Definitions applicable to Section 1 of the policy to which this endorsement attaches. Identity Fraud means the act or acts of knowingly transferring or using, without lawful consent or authority, your means of identity which constitutes a violation of any federal, provincial, territorial or municipal law. Identity Fraud does not include the fraudulent alteration of account profile information, such as the address to which statements are sent. Identity Fraud does not include the unauthorized use of valid credit cards, credit accounts or bank accounts. Identity Fraud does not include the fraudulent use of a business name, trade name or brand identity or other method of identifying a business activity. Identity Fraud Occurrence means any act or series of acts of Identity Fraud by a person or group which results in an insured loss during the policy period. All other definitions applicable to the policy to which this endorsement form is attached are applicable. Coverage We insure, to the amount as stated on the Declaration Page in total during the term of this policy, the following reasonable costs and expenses incurred by an insured person, as a result of an Identity Fraud Occurrence: 1. Reasonable costs associated with registered mail to businesses, law enforcement agencies, financial institutions, credit agencies and similar credit grantors. 2. Fees for the re-application of loans which had been declined as a result of incorrect or erroneous information; 3. The reasonable costs or expenses (including mileage, associated parking costs, taxi cab fees or public transit fees) incurred for notarizing affidavits for financial institutions, credit agencies, credit grantors or similar lenders. 4. The reasonable costs or expenses (including mileage, associated parking costs, taxi cab fees or public transit fees) incurred for notarizing affidavits for law enforcement agencies. 5. Long distance telephone expenses to discuss an actual Identity Fraud Occurrence to businesses, law enforcement agencies, financial institutions, credit agencies and similar credit grantors EO August 2012 Page 1 of 3

Earnings lost resulting from necessary time away from your employment for the purposes of completing affidavits and meeting with credit agencies, similar credit grantors, law enforcement departments, financial institutions, merchants and legal counsel, up to the amount as stated on the Declaration Page per day to a maximum amount as stated on the Declaration Page, for each Identity Fraud Occurrence. 7. Reasonable costs, fees or expenses associated with the replacing of Canadian or Provincial Government issued documents as a result of an Identity Fraud Occurrence. 8. Reasonable legal fees incurred directly as a result of an Identity Fraud Occurrence, with prior notice to us for: a) The removal of any criminal or civil judgements wrongly entered against you. b) To challenge the information in your consumer credit report. c) The defence of lawsuits brought against you by businesses or their collection agencies. 9. We will reimburse you for the reasonable cost of obtaining up to two credit reports after an Identity Fraud Occurrence has been reported to us, for a period of up to 12 months from the date of the reporting of the occurrence, but not limited by the expiry date of the policy. Loss or Damage Not Insured We do not insure: 1. your fraudulent, dishonest, or criminal acts; 2. your own use of your identity; 3. your commercial or business pursuits; 4. your intentional misuse of your identity; 5. fraudulent, dishonest, criminal or intentional misuse of your identity by any resident of your household; Nor do we insure the following: 6. Any losses covered under the Credit or Debit Cards, Automated Teller Cards, Library or Video Cards, Forgery and Counterfeit Money coverage already available in the underlying policy; 7. Any losses covered by credit card insurance, bank insurance or other coverage available to you. This endorsement will be secondary with other insurance being primary. This endorsement will only apply once the other insurance available to you has been exhausted. Deductible In any one occurrence, we are responsible only for the amount by which the insured loss or damage exceeds the amount of the deductible shown on the Declaration Page. If one occurrence could lead to the application of more than one deductible, only the largest deductible will apply. EO August 2012 Page 2 of 3
3 Notice to Authorities In addition to the requirements outlined on the policy to which this endorsement attaches, you are required to contact your local law enforcement agency of the Identity Fraud Occurrence. All other policy terms, limits and conditions apply. EO August 2012 Page 3 of 3
4 VERS. 05/2012 What You Need to Know About Identity Fraud IDENTITY FRAUD PROTECTION 1. Identity fraud can occur over the Internet or telephone, or via fax or regular mail. Therefore, be particularly wary of unsolicited s, telephone calls or mail attempting to extract personal or financial information from you. 2. When you change your address, make sure you notify the post office and all relevant financial institutions (your bank and credit card companies). 3. Carry only the identification information and credit cards that you actually require. Do not carry your social insurance number (SIN); leave it in a secure place. This also applies to your passport and other travel documents such as International Student Identity Cards. 4. Pay attention to your billing cycles and follow up with your creditors and utility companies if your bills do not arrive as scheduled. 5. Periodically check your credit reports, bank and credit card statements and report any irregularities promptly to the relevant financial institution and to the credit bureaus. 6. Destroy paperwork you no longer need, such as banking machine receipts, utility bills and credit card receipts. Destroy pre-approved credit card applications you do not need before you dispose of them. 7. During transactions, it s safer to swipe your cards yourself than it is to allow a cashier to do it for you. If you must hand over your card, never lose sight of it. Always shield your personal identification number when using an ATM or a PIN pad. 8. Never give personal information over the phone, through the mail or over the Internet unless you initiated the contact and are familiar with the organization or person with whom you are dealing. 9. Password-protect your credit card, bank and phone accounts. Avoid using easily determined PIN numbers and never keep a record of them with you. Change your computer password regularly or as prompted by the operating system. 10. When using the Internet, take security precautions by using encryption, digital signatures, and protected payment methods. Install a firewall and/or other anti-virus software to protect your computer and wireless connections from unauthorized users and viruses. 11. Order a copy of your credit report from a major creditreporting agency at least once a year. Make sure your credit report is accurate and includes only those activities that you authorized. SUSPECT IDENTITY FRAUD? Recommendations from RCMP Website If you suspect or know that you are a victim of identity theft or fraud, or if you unwittingly provided personal information or financial information: Step 1: Contact your local police force and file a report. Step 2 : Contact your bank/financial institution and credit card company Step 3: Contact the two national credit bureaus and place a fraud alert on your credit reports. o o Equifax Canada Toll free: TransUnion Canada Toll free: Step 4: Always report identity theft and fraud. Contact the Canadian Anti-Fraud Centre
5 THE IDENTITY FRAUD CHECKLIST If you suspect you are a victim of Identity Fraud please refer to the following: Report the crime to the local police as soon as possible. Be sure to obtain a copy of the police report for future reference. Also notify the RCMP phonebuster unit at Notify all creditors and financial institutions with which you have dealings such as Banks, Credit Card Companies, Utilities, Lending Firms, and Retailers etc. Be cautious of hiring a credit repair firm prior to checking with the Better Business as to their legitimacy. Cancel your credit cards and have new cards issued. Explain in detail the reasons for this and inquire about accounts opened fraudulently in your name. Cancel all ATM cards and have new ones with new PIN numbers issued. Destroy your cheque books and have new and different cheques issued. Cancel any telephone calling cards. Follow up with Canada Post in the event you think your mail is being tampered with or diverted. Follow up with your local land registry office to confirm ownership of your property and whether your identity has been used to register other properties. Obtain a new driver s license and health card. Obtain a credit report and have it noted to reflect an ID Fraud Occurrence. Follow up with a further report to insure your ID isn t being used again. Obtain a new birth certificate if necessary. Obtain a new Canadian passport, Status Card, Canadian Citizenship Certificate, Immigration Record of Landing or Permanent Resident Card if necessary. Change your password on all accounts where a password is required. If you suspect your SIN number is being used contact Human Resources Development Canada at: Social Insurance Registration P.O.
