1 CARD INSURANCE POLICY I. DEFINITIONS a. Cardholders means the type of Cardholders listed under beneficiaries section, whose credit card account is valid and in good standing, not in delinquency, collection, or cancellation status. b. Chartis Vietnam means the company providing this insurance c. Relative means Cardholder s legally married spouse, parents, step-parents, parents in-law, grandparent, child, stepchild, legally adopted child, grandchild, brother, brother in-law, sister, sister in-law, son in-law, daughter in-law, uncle, aunt, niece, nephew, and first cousin who is/are normally resident in Cardholder s Residence. d. Money means currency, coins and bank notes in current use in Vietnam and having a face value. e. Payment card means the ATM cards, the credit cards issued by Citibank for personal use only, which are listed under Beneficiaries section. f. ATM means automatic teller machines using Citibank payment cards worldwide. g. Lost or Stolen means having been inadvertently lost or having been stolen by a third party without Cardholder s assistance, consent or co-operation. h. Residence means the place where the Cardholder permanently stays in Vietnam, and it is used solely for residential purpose. i. Business means: 1. A trade, profession or occupation engaged in on a full-time, part-time or occasional basis; or 2. Any other activity engaged in for money or other compensation. j. Robbery means the unlawful taking of money or other property from Cardholder s care and custody by one who has caused or threatened Cardholder with bodily harm and has committed an illegal or violent act. k. Bodily Injury means bodily harm, sickness or disease, which results in required care, loss of services or death of the Cardholder.
2 II. SCOPE OF COVER 1. What Chartis Vietnam covers A. ATM ASSAULT AND ROBBERY a) ATM Assault and Robbery Chartis Vietnam will reimburse Cardholder for the money Cardholder withdrew from any ATM in Viet Nam using Citibank payment card against a robbery event that occurs within fifteen (15) minutes of the withdrawal of the money. b) Bodily Injury Chartis Vietnam will reimburse Cardholder for reasonable emergency first aid charges for bodily injury during a robbery that is covered by Chartis Vietnam ATM Assault and Robbery coverage. 2. Coverage Exclusions Chartis Vietnam will not pay for any: 1. damages and/or liabilities to any third parties; 2. damages or losses to anything other than the money Cardholder withdrew from Cardholder account; 3. damages and/or liabilities that happened before or after the covered robbery period; 4. charges for emergency first aid to anyone other than Cardholder 3. Coverage Conditions a) Cardholder must provide an official police report that indicates the incident happened within the covered time frame in order for Chartis Vietnam to pay the claim. b) Card with the card number being changed during the policy period, the Cardholder still be covered. Citibank will confirm with Chartis Vietnam about these changes, which will be taken as the evidence for claim settlement for Cardholder when event occurs. 4. Documentary Requirement of ATM Assault and Robbery Please note that the documents listed below are not all inclusive and additional documents may be required in the course of processing the claims. a) Duly completed claims form b) A copy of police report to verify the event c) Bank ATM withdrawal slip or Bank statement of passbook d) A copy of your identity card MEDICAL REIMBURSEMENT FOR INJURY DUE TO ROBBERY OR ASSAULT a) Duly completed claim form b) A copy of your identity card c) Police report d) Original detailed medical bill (with break down of charges) e) Original medical receipt Contact address of Claims Department: Chartis Vietnam Insurance Company Limited - Claim Department :

Tel: Fax: Chartis Vietnam will disburse payment on valid claims to the Cardholder within fifteen (15) days after receiving the all required documents. 1. What Chartis Vietnam cover B. ENHANCED FRAUDULENT PROTECTION a) If the payment card is lost or stolen, Chartis Vietnam will reimburse the unauthorized charges that Cardholder is responsible for on Cardholder lost or stolen payment card, up to twelve (12) hours prior to and twelve (12) hours after Cardholder first reporting the event to Citibank; or b) If your payment card was not lost or stolen, but unauthorized charges were made through ATM withdraw, in-store or online purchases with your payment card information, Chartis Vietnam will reimburse the said unauthorized charges made up to 2 month (60 days) prior to Cardholder first reporting the event to Citibank or Citibank notifies Cardholder about the event (whichever occurs first). 2. Coverage Exclusions Chartis Vietnam will not pay for any expenses or loss for: a) Cash advances made with Cardholder s lost or stolen payment card; b) Charges incurred by a resident of Cardholder household, or by a person entrusted with Cardholders payment card. c) Chartis Vietnam will not pay the cancellation and/ or replacement fee of the covered card types in case the payment card is lost, stolen or the card information have been captured illegally and unauthorized charges have been made against them. 3. Coverage Conditions a) Chartis Vietnam will only pay for unauthorized charges for which Cardholder is responsible under the terms and conditions of the payment card. b) Cardholder must report the loss or theft of the payment card to the issuer(s) and to Chartis Vietnam within twenty (24) hours after discovering Cardholder s lost or stolen payment card event. c) Cardholder must comply with all terms and conditions by which Cardholder payment card is issued. d) For Cover #2 of Section 1, during the first two (02) months, we will only cover the unauthorized charges that were incurred after the policy effective date. e) In the event the covered card has been lost or stolen or the card information has been captured illegally and unauthorized charges have been made against it then the card has to be cancelled and replaced by new by Citibank. 4. Duties After An Accident or Loss In the event of a covered loss: Case 1: If the payment card is lost or stolen (incurred the unauthorized charges before or after the time which Cardholder report to Citibank):
4 Claim procedure : a) Step 1: Cardholder calls Citibank or Chartis Vietnam at immediately after discovering his payment Card is lost or stolen b) Step 2: Citibank block/ temporarily block the lost or stolen payment card immediately after it receives the call from the Cardholder or Chartis Vietnam c) Step 3: Citibank checks and confirms if there are any unauthorized charges incurred before and after the Cardholder s reporting of the lost or stolen card; and the unauthorized charges are responsible by the Cardholder d) Step 4: Citibank advises the Cardholder about Chartis Vietnam s claims hotline to report the claim. Chartis Vietnam will contact Citibank /Cardholder for necessary supporting documents, including but not limited to the ones listed below e) Step 5: Chartis Vietnam checks required documents and will disburse the payment to the Citibank on the valid claim within fifteen (15) days after receiving the required documents. Documents required: a) Duly completed claims form b) The reporting time of lost/stolen card from the Cardholder to Citibank in writing: The record of the first time that Cardholder request to block the card (for lost / stolen card) with information of card number c) Copy of Billing statements which show the fraudulent transaction d) A copy of police report to verify the stolen payment card event. e) The claims form and accompanied documents must be returned to Chartis Vietnam within thirty (30) days of making the original claims. f) Acquirer Bank s receipt, according on purchase method and specific Acquirer Bank. g) Chargeback document which show that Acquirer deny to pay for customer. Case 2 : If the payment card was not lost or stolen, and there were unauthorized charges Claim procedure : a) Step 1: Cardholder receives the billing statement and notifies Citibank immediately about unauthorized charges or Citibank discovers unauthorized charges and immediately notifies Cardholder b) Step 2: Citibank block / temporarily block the payment card that has the unauthorized charges immediately after the confirmation from the Cardholder c) Step 3: Citibank advises the Cardholder about Chartis Vietnam s claims hotline to report the claim. Chartis Vietnam will contact Citibank /Cardholder for necessary supporting documents, including but not limited to the ones listed below. d) Step 4: Chartis Vietnam checks required documents and will disburse the payment to Citibank within fifteen (15) days after receiving the required documents. Documents required: a) Duly completed claims form b) The reporting time of unauthorized charges from the Cardholder to Citibank in writing and a copy of the recording tape of the Cardholder reporting of unauthorized charges to Citibank (if available) upon request. c) Copy of Billing statements which show the fraudulent transaction: the list of unauthorized transactions occured within two (02) months before the first time the Cardholder reported to Citibank or Citibank notified the Cardholder about the unauthorized charges and other documents relating to the unauthorized charges. d) The claims form and accompanied documents must be returned to Chartis Vietnam within
5 thirty (30) days of making the original claims. e) Acquirer Bank s receipt, according on purchase method and specific Acquirer Bank. f) Chargeback document which show that Acquirer deny to pay for customer. III. CLAIMS PROCEDURE In the event of an occurrence of a claim, Cardholders shall: 1. Report to the Police within twenty-four (24) hours from time of the occurrence involving robbery or theft. 2. Immediately notify Chartis Vietnam/ Citibank by telephone of the event/ loss (see contact details below), provide following information : a) Time and Date of the event b) The Cardholder s name and the Cardholder s number. c) Telephone number of Cardholder (this is to enable us to fix an appointment or to obtain more information on the claim to decide the next course of action) d) Brief description how the event occurred or how the loss or damaged was discovered e) Name of the items damaged or stolen (if any) 3. Chartis Vietnam/Citibank will provide Cardholder with a claim form or an acknowledgment of Cardholder s notification 4. A list of documents will be requested by Chartis Vietnam with the claim form. The required document must be submitted to Chartis Vietnam within thirty (30) days of making the original claim. 5. Appointment of Adjuster: At the discretion of Chartis Vietnam if necessary. 6. Chartis Vietnam will disburse payment on valid claims to the Cardholder within fifteen (15) days after receiving the all required documents. 7. In the case of Chartis Vietnam will require Citibank s assistance in confirming the status of the Cardholder in the case of a claim incurred before Chartis Vietnam receives Cardholders details, Citibank will provide necessary assistance to Chartis Vietnam within the reasonable timeline. IV. GENERAL EXCLUSIONS Chartis Vietnam will not cover the following: 1. Losses that do not occur within this Policy period; 2. Losses that result from or related to business pursuits including Cardholder work or profession; 3. Losses caused by illegal acts against Vietnam applicable laws; 4. Losses that Cardholder has intentionally caused; 5. Losses that result from the direct actions of a relative, or actions that a relative knew of or planned. 6. Losses due to war, invasion, act of foreign enemy, hostilities or warlike operations (whether war has been declared or not), civil war, rebellion, revolution, insurrection, civil commotion, uprising, military or usurped power, martial law, terrorism, riot or the act of any lawfully constituted authority. 7. Losses due to the order of any government, public authority, or customers officials. 8. Losses from the Cardholders: i. For whom Cardholders did not pay the premium V. GENERAL CONDITIONS 1. Arbitration, governaned laws
6 Any dispute arising under or in connection with this contract shall be resolved by Chartis Vietnam and Cardholder through amicable settlement. If both parties fail to resolve amicably within thirty (30) days from the date of notice on dispute, the dispute shall be referred to the Vietnam International Arbitration Centre at the Vietnam Chamber of Commerce and Industry in accordance with its rules, or another arbitration body and rules at the option of the Insurer, for final settlement. The dispute shall be referred to the competent court for settlement if the choice of arbitration shall not be available between the parties under the applicable law. 2. Valid Account Cardholders account must be valid and in good standing for coverage to apply. Benefits will not be paid if, on the date of occurrence, on the date of claim filing, or on the date of would-be claim payment, Cardholders payment card account is in delinquency, collection, or cancellation status. 3. Excess of Other Insurance Coverage Coverage provided by this Policy are EXCESS; this means that if, at the time of occurrence: (1) Cardholders have other valid and collectible insurance - such as but not limited to homeowner's or renter's insurance this Policy will only cover that amount not covered by such other insurance, up to the limits of the specific coverage. (2) If the event is covered by more than one of the coverage within this Policy, Chartis Vietnam will only pay the amount from the coverage under which Cardholders first filed the claim. 4. Law and Practice This Policy shall be governed by the law of Vietnam 5. Policy Period The maximum policy period for this Policy is one (01) year 6. Concealment or Fraud The coverage will be voided, whether before or after the loss, if Cardholder willfully concealed or misrepresented any material fact or circumstance concerning this insurance or provided fraudulent information to Chartis Vietnam. Cardholders must use all reasonable means to avoid future loss at and after the time of loss. 7. Duties After an Accident or Loss Chartis Vietnam has no duty to provide coverage under this policy unless there has been full compliance with the duties that are detailed in each coverage section. Cardholders are required to cooperate with Chartis Vietnam in investigating, evaluating and settling a claim. 8. Our Cancellation Chartis Vietnam may cancel this policy or any part of it at any time by giving Cardholders sixty (60) days prior written notice. When this Policy is canceled, the premium for the period from the date of cancellation to the expiration date will be refunded using prorate methodology listed below. Cardholders Card Cancellation In the event that the Cardholder cancels the covered Payment card during the coverage period, the premium for the period from the date of cancellation to the payment card expiration date will be refunded using the prorate methodology mentioned above. 10. Subrogation If Chartis Vietnam shall make any payment or otherwise make good any loss applying under this Policy, it shall be subrogated to all the rights of recovery against any other person or persons and shall complete, sign and deliver any documents necessary to secure such rights. Cardholder shall not take any action following a loss to prejudice such rights of subrogation. 11. Burden of Proof In any action, suit or other proceedings where the Chartis Vietnam alleges that by reason of the provisions of any exclusion which may be applicable, any loss, destruction or damage is not covered by this policy, the burden of proving that such loss, destruction or damage is covered shall be upon the Cardholders. 12. Sanction Clause Chartis Vietnam is not liable to make any payments for liability under any coverage sections of this Policy or make any payments under any extension for any Claim arising in, or where any beneficiary under the Policy is a citizen or instrumentality of the government of, any country/countries against which any laws and/or regulations governing this Policy and/or Chartis Vietnam, its parent company or its ultimate holding entity have established an embargo or other form of economic sanction which have the effect of prohibiting Chartis Vietnam from providing insurance coverage or transacting business with or otherwise offering economic benefits to the Cardholders or any other beneficiary under the Policy. It is further understood and agreed that no benefits or payments will be made to any beneficiary/beneficiaries who is/are declared unable to receive economic benefits under the laws and/or regulations governing this Policy and/or the Chartis Vietnam, its parent company or its ultimate holding entity.
